Winter 2018-19 Athletic Forms

September 2018

We're providing this information in advance so that you can schedule a physical with your child's physician if he/she is due.  The items we need are explained below:

Bishop McHugh students wishing to participate in our winter sports programs (basketball or cheerleading) must be given a pre-participation physical examination pursuant to N.J.A.C. 6A:16.  Basketball is for boys and girls in grades 3-8 and cheerleading is for girls in PK4 - grade 2.  Physical exams are good for one calendar year.  Although the Athletic Physical Forms are required for grades 6-8, it's most convenient that we use them for all grades (instead of crafting our own forms for grades 5 and below).

A child's health history must be updated every 90 days (via the update form completed by parent/ guardian.)  In other words, even if you child has had a physical within the past year, please complete and turn in the one-page health history update form prior to the winter season. This alerts us to any medical updates since the last physical.

Only the health history form, the physical exam forms and signature form (e.g., Concussion Fact Sheet) are to be turned in.  Informational pamphlets, such as Sports Related Eye Injuries and Cardiac Information are yours to keep.  It is required that we distribute these to you.

Finally, please fill out the paperwork completely and legibly so that it can be processed in an organized fashion. Incomplete forms cannot be accepted and may result in your child being ineligible.
